Please report to the main hospital Admitting Department on the first floor one hour before your scheduled procedure time.  Please bring your insurance cards and a list of any medications that you take at home.  You MUST bring a responsible person with you to drive you home. 

Please report to the Seminole Ambulatory Care Center, Room 208, on the second floor of the Seminole Ambulatory Care Center one-half hour before your scheduled procedure time.  The Seminole Center is located on the corner of  West Huron and Seminole Streets.  You DO NOT need to go to the main hospital Admitting Office prior to your exam.  Free parking is available on Seminole Street, across the street from the entrance to the Seminole Center.  Please bring your insurance cards and a list of any medications that you take at home.  You MUST bring a responsible person with you to drive you home.
