New Patient History Form
If you are a new patient, you can avoid a delay in the office by downloading
our New Patient History Form on-line, filling it out at home and
bringing it with you when you come for your first appointment. Or,
if you prefer, you can fax it to our office at (248) 682-4909 any time
prior to your appointment. To download this file you will need Adobe
Acrobat Reader. If you don't have it on your computer you can download
it for free by clicking the link below. Click the "Back" button on
your browser to return to this page after downloading and installing Acrobat
Reader. If you already have Acrobat Reader installed on your computer,
you are ready to download and print out the New Patient
History Form.
